Current treatment guidelines recommend using Peg-interferon and Ribavirin along with Boceprevir or Telaprevir for genotype 1. For genotype 2 & 3, Peg-interferon and Ribavirin is used. Sofosbuvir (Sovaldi) is a new drug for treatment of Hepatitis C and is made by Gilead. It got approved for Hepatitis C in December 2013. It is a nucleotide analogue inhibitor of NS5B polymerase enzyme which is essential for viral replication. Sofosbuvir is a pro drug which undergoes intracellular metabolism to an active triphosphate form and gets incorporated into viral RNA and acts as a chain terminator. Common side effects from Sofosbuvir are rash, diarrhea, anemia, and nausea. It also has some rare but serious side effects i.e. pancytopenia, increased bilirubin levels and depression. Sofosbuvir is a category B medication in pregnancy. It is available as a 400 mg tablets and is given once daily. Some advantages of Sofosbuvir are once daily dosing, given orally and shorter treatment duration. Sofosbuvir is a very expensive medication with an average wholesale price of around $1,200 per tablet.
